Specimen Collection Criteria

Collect (preferred specimen): One Gold-top SST tube.
Also acceptable: One plain Red-top tube.

Physician Office/Draw Specimen Preparation

Let specimen clot 30-60 minutes then centrifuge to separate serum from cells. Transfer serum to a plastic transport tube and refrigerate (2-8°C or 36-46°F).

Preparation for Courier Transport

Transport: 0.2 mL serum, refrigerated (2-8°C or 36-46°F). (Minimum: 0.1 mL)

Rejection Criteria

Specimens not collected and processed as indicated.

In-Lab Processing

Clinical Utility

This quantitative assay uses Brugia malayi microfilaria antigen to detect filaria-specific IgG and IgM antibodies. Detection of B. malayi-specific total IgG is highly sensitive but is not specific due to the cross-reactivity with other helminths or certain protozoa that share a common antigen of phosphocholine (PC). Detection of filarial IgM and a significant rise in filarial IgG antibody between an acute (drawn at the onset of illness) and convalescent (drawn 3-6 weeks later) specimen is the best evidence of a recent infection. Detection of B.malayi-reactive IgG4 improves the specificity of detection of filarial antibodies.
