Instructions

A number of drugs are known to influence endocrine function directly, or to interfere with assay methodology. Whenever possible, physicians should consider discontinuing all drugs, except those necessary to the patient's well-being, several days prior to sample collection. Always note any drugs being administered on the Test Request Form.

Specimen Collection Criteria

Collect (preferred specimen): One plain Red-top tube.
Also acceptable: One Gold-top SST tube.

Physician Office/Draw Specimen Preparation

Let specimen clot 30-60 minutes then centrifuge to separate serum from cells within one hour of collection. Transfer serum to a plastic transport tube and refrigerate (2-8°C or 36-46°F).

Preparation for Courier Transport

Transport: 0.5 mL serum, refrigerated (2-8°C or 36-46°F). (Minimum: 0.4 mL)

Rejection Criteria

Specimens not collected and processed as indicated.

In-Lab Processing

Clinical Utility

Plasma DOC concentration is increased in congenital adrenal hyperplasia caused by ß-hydroxylase or 17-hydroxylase deficiency. This assay is used to evaluate virilization and /or hypertension. It also aids in the assessment of adrenal tumors with endocrine activity. Plasma DOC levels may be increased in some patients with adrenal carcinoma, and a DOC-secreting carcinoma has been described.
