Amiodarone Level

Specimen Collection Criteria

Collect: One plain Red-top tube. (Minimum Whole Blood: 2.0 mL)

Do not use Serum Separator Tubes.
Time of Collection:
 Trough, just prior to the next dose.
Record the exact time of specimen collection on the tube or in the computer system.

Physician Office/Drawsite Specimen Preparation

Let specimens clot 30-60 minutes. Centrifuge specimen to separate serum from cells. Refrigerate (2-8°C or 36-46°F) the centrifuged collection tube within two hours of collection. (Min: 1.0 mL serum)

Preparation for Courier Transport

Transport: Centrifuged collection tube, refrigerated (2-8°C or 36-46°F). (Min: 1.0 mL serum)

Rejection Criteria

  • Serum Separator (SST) tubes.
  • Severely lipemic or hemolyzed specimens.

Storage

Red-top tubes may be stored for up to 24 hours at refrigerator temperatures (2o -8oC). DO NOT FREEZE TUBES CONTAINING BLOOD. For longer storage, the serum should be removed from the clot and refrigerated (2o-8oC) or frozen at -20o C or below.

Serum specimens (pour-overs) refrigerator temperature (2o to 8oC; 36o to 47o F) for 24 hours, and at -20oC (-4o F) for up to 3 months. Specimens stored at -70o C (-94o F) can be stored indefinitely.

Performed

Tuesday, Friday.
Results available the same day as testing performed (1-4 days).

Reference Range

Therapeutic Range: 1.0-2.5 mcg/mL. (No therapeutic range exisits for Amiodarone plus Desethylamiodarone.)
Critical Level: Greater than 5.0 mcg/mL.

Test Methodology

High Performance Liquid Chromatography with Ultraviolet Detection (HPLC/UV).

This test was developed and its performance characteristics determined by Beaumont Health. It has not been cleared or approved by the FDA. The Laboratory is regulated under CLIA as qualified to perform high-complexity testing. This test is used for clinical purposes and should not be regarded as investigational or for research.

Interpretation

The half-life of a Amiodarone may be 30 days or more. Drug accumulation can result in toxicity.
The concentration of desethylamiodarone, a major metabolite, is also reported, but no therapeutic range is available. This metabolite may be pharmacologically active, but the extent of activity is not currently known with certainty.

Clinical Utility

Amiodarone (Cordarone) is an antiarrhythmic drug. This assay is used to monitor the therapeutic drug level and to evaluate potential cases of Amiodarone toxicity.
