Become a Client
Hospitals, clinics, and commercial laboratories that would like to refer laboratory testing to Beaumont Laboratory should submit a Client Request Form. The completed form can be emailed or faxed to 248-551-1151 and one of our representatives will contact you. For your reference, the following is a list of important information for new clients:
- Ordering First Test - Clients must have an account to submit online orders through Beaumont Labworks. If you are not currently set up with an account, please call your Business Development Representative or Customer Service at 800-551-0488. If this is the first time you are requesting a test from Beaumont Laboratories, please visit our Ordering First Test page, which provides step-by-step instructions.

- Reporting Options - Please discuss the varied reporting options with your sales representative. They will work with you to choose your best option.

Manual orders are for clients who are not configured to submit online orders or who normally use paper forms. Please call Customer Service at 800-551-0488 for a complete list of requisitions.
Test Add-On Request
To request additional tests after a specimen has been shipped, please call customer service at 800-551-0488.
Quality Assurance and Accreditation

Referring clinics, hospitals, laboratories, and/or physicians may establish institution accounts with convenient monthly invoicing and payment options. A completed Client Request Form or Laboratory Services Agreement must be on file in the billing office for account approval.
