Specimen Collection Criteria

Collect (preferred specimen): One Gold-top SST tube.
Also acceptable: One plain Red-top SST tube.

Send specimen for processing immediately after collection.

Physician Office/Drawsite Specimen Preparation

Let specimen clot 30-60 minutes then centrifuge to separate serum from cells. Transfer serum to a plastic transport tube and freeze (-20°C/-4°F or below) immediately.

Preparation for Courier Transport

Transport: 1.0 mL serum, rfreeze (-20°C/-4°F or below). (Min: 0.1 mL)

Rejection Criteria

  • Grossly hemolyzed specimens.
  • Lipemic specimens.
  • Specimens not collected and processed as indicated.

Storage

Specimen Stability for Testing:

Room Temperature (20-26°C or 68-78.8°F): 8 hours
Refrigerated (2-8°C or 36-46°F): 48 hours
Frozen (-20°C/-4°F or below): 14 days

Reference Range

Negative: Less than or equal to 20 Units.
Weakly Positive: 21-30 Units.
Positive: Greater than 30 Units.

Test Methodology

Enzyme-Linked Immunosorbent Assay (ELISA).

Interpretation

A positive result has a high predictive value for microscopic polyangitis (MPA) in patients with a negative test result for systemic lupus erythematosus (SLE) and Goodpastures syndrome. A negative result significantly diminishes the likelihood that the paitent has MPA.

MPO levels often decline following successful treatment of MPA; but, specific guidelines for this clinical purpose are not available.

Clinical Utility

Useful for evaluating patients suspected of having immune mediated vasculitis, especially microscopic polyangitis (MPA).

May be useful to follow treatment response or to monitor disease activity in patients with MPA.
