Specimen Collection Criteria
Collect (preferred specimen): One Gold-top SST tube.
Also acceptable: One plain Red-top tube.
Physician Office/Drawsite Specimen Preparation
Let specimen clot 30-60 minutes then centrifuge to separate serum from cells within one hour of collection. Transfer serum to a plastic transport tube and freeze (-20°C/-4°F or below).
Preparation for Courier Transport
Transport: 1.0 mL serum, frozen (-20°C/-4°F or below). (Min: 0.2 mL)

Specimen Stability for Testing:
Room Temperature (20-26°C or 68-78.8°F): 14 days
Refrigerated (2-8°C or 36-46°F): 14 days

Circulating insulin-like growth factor-1 (IGF-1) is almost totally bound to high affinity binding proteins called IGF binding proteins, (IGFBPs). Six IGFBPs have been identified, IGFBP3 is the major carrier of IGFs in serum and is present in the highest concentrations. More than 90% of IGF-1 and IGF-2 is bound to IGFBP3 and the complex in humans has a half life of 12-15 hours.
IGFBP3 may provide a storage pool for IGF-1 and may be involved in transport of IGF-1 to tissues. Serum concentrations of IGFBP3 are fairly constant throughout the day and are controlled by GH and IGF-1 levels.
The measurement of IGFBP3 is useful in the evaluation of several clinical situations:
- short stature in children.
- nutritional status.
