Instructions

Specimen Collection Criteria

Collect (preferred specimen): 24 hour urine sample with no preservative.
Also acceptable: Random urine specimen in a screw-capped container (preferred) or other sterile collection cup. (Min: 1.0 mL)

  • Keep 24 hour urine specimen iced or refrigerated during collection.
  • Include start and end dates and times for the collection period on the specimen container.

Physician Office/Drawsite Specimen Preparation

Maintain 24 hour urine specimens refrigerated (2-8°C or 36-46°F) and freeze (-20°C/-4°F or below) random urine specimens prior to transport.

Preparation for Courier Transport

Transport: Entire 24 hour urine collection, refrigerated (2-8°C or 36-46°F), or random urine, frozen (-20°C/-4°F or below).

Rejection Criteria

  • Specimens not collected and processed as indicated.

Specimen Stability for Testing:

Room temperature (20-26°C or 68-78.8°F): Unacceptable
Refrigerated (2-8°C or 36-46°F): 4 days
Frozen (-20°C/-4°F or below): 1 month

Clinical Utility

Elevated urinary methylmalonic acid is an early and sensitive indicator of vitamin B12 (cobalamin) deficiency. Also used to monitor patients with methylmalonic aciduria; diagnosis of methylmalonic aciduria requires an organic acid panel and appropriate clinical history.
