Specimen Collection Criteria

Collect: 50 mL random urine in a screw-capped container (preferred) or other sterile collection cup. (Minimum: 10 mL)

Specimens should be delivered to the Laboratory within one hour of collection to prevent leukocyte lysis.

Physician Office/Draw Specimen Preparation

Arrange for transportation to the Laboratory prior to specimen collection. Maintain specimens refrigerated (2-8°C or 36-46°F) prior to transport.

Preparation for Courier Transport

Transport: 50 mL urine, refrigerated (2-8°C or 36-46°F). (Minimum: 10 mL)

Rejection Criteria

  • Specimens exceeding recommended time limits (one hour at room temperature, three hours refrigerated).
  • Specimens with low cell counts or unacceptable volume may give unsatisfactory results but testing will still be attempted.

Inpatient Specimen Preparation

Transport specimen to the Laboratory immediately after collection for processing.

Frozen (-20°C/-4°F or below): Unacceptable

Test Methodology

Cytocentrifuge smears of urine sediment and the Prussian-Blue stain.

Interpretation

Hemosiderin is present in diseases involving a true siderosis of kidney parenchyma (hemochromatosis). It is also present 2-3 days after an acute hemolytic episode that produces hemoglobinemia and hemoglobinuria. Hemosiderin granules are found in intact renal tubular epithelial cells or occasionally in casts and may also be seen extracellularly.

Clinical Utility

This assay aids in the detection of hemochromatosis and hemoglobinuria. Hemosiderin is typically found in urine of acid and neutral pH, but not alkaline pH.
