Specimen Collection Criteria

Collect (preferred specimen): Random urine specimen in a screw-capped container (preferred) or other sterile collection cup. (Minimum: 5.0 mL)

  • Refer to the table of Urine Preservative Options when multiple tests are requested.
  • Include start and end dates and times for the collection period on the specimen container.

Physician Office/Draw Specimen Preparation

Maintain specimens refrigerated (2-8°C or 36-46°F) prior to transport.

Preparation for Courier Transport

Transport: Random urine, refrigerated (2-8°C or 36-46°F).

Rejection Criteria

  • Urines collected with Acetic Acid or Sodium Carbonate.
  • Specimens not collected and processed as indicated.

In-Lab Processing

Aliquot a minimum of 5.0 mL from the random urine.

Storage

Specimen Stability for Testing:

Room temperature (20-26°C or 68-78.8°F): 4 hours
Refrigerated (2-8°C or 36-46°F): 7 days
Frozen (-20°C/-4°F or below): For longer storage.

Specimen Storage in Department Prior to Disposal:

Refrigerated (2-8°C or 36-46°F): 7 days

Reference Range

0.02-0.26 (both calcium and creatinine reported as mg/mg creatinine).

Test Methodology

Spectrophotometry.

Interpretation

This assay includes a Urine Calcium and a Urine Creatinine assay.

Clinical Utility

The urine calcium/creatinine ratio is useful as a screening test to assess the presence or absence of hypercalciuria. Ideally a 24 hour urine for calcium is the best indicator of this condition, but a random urine is more convenient for patients, especially children. Results greater than 0.26 suggest hypercalciuria.
